經由順磁性藥物在微血管與組織間交換的動態影像,可以提供關於腦組織在血液動力學方面的資料;而此血液在腦組織間之交換機轉可以用來建立描述局部腦血容積的功能影像。因為血液在腦組織與微血管間之流通往往在數秒內完成,因此我們需要快速擷取影像技術來解析與測量血管內染劑在微血管層與腦組織間轉換的訊息。 我們使用GE公司4.7 Tesla的Omega CSI振影像系統,以SE波序來做貓腦缺血缺氧的動物實驗。整個實驗模型乃夾住左下鎖骨大動脈及臂頭徑大動脈,隨之一段時間再鬆弛,使腦血流重新灌注。在腦缺血缺氧一段時間後,發現有極明顯之腦血流增加(充血),並可由血流放射微粒同位素觀察得到佐證;同時合併做三叉神經切除時可發現同側腦血流充血反應的降低。此研究的目的在4.7 T的磁振儀器下來探討偵測腦缺血缺氧的特性、血流重新灌注的充血反應及其對三叉神經切除的影響。有關偵測腦缺血缺氧及其重新灌注血流之動態磁振影像,初步所得的資料與放射性微粒在血流動力短暫的變化是一致的,此也說明對腦缺血缺氧的瞭解是基於血流灌注的變化,而不同於腦組織水腫的瞭解。當然在測量血流動力學磁振影像時,仍然有其它因素需要加以考慮,其中最重要的是要能夠在極短的時間內獲取影像,例如必須在極短的回音及波序時間內完成影像。此外,像動物實驗需要縮小視野區以獲得較強的磁場梯度,諸如此類的考慮及技術,都是在磁振影像上完成腦組織血流動力研究所必備的條件。
Dynamic imaging of the cerebral transit of paramagnetic contrast agent provides information about the hemodynamics state of brain tissue. Functional images depicting regional cerebral blood volume are generated from the transit-time data. Because cerebral transit times are on the order of seconds, rapid imaging techniques are required to resolve and measure the passage of an intravascular agent through the capillary bed. Gradient echo imaging sequences on the 4.7 Tesla Omega CSI GE Imager are used to measure cerebral transit times in a cat experimental ischemia model. This global model involves occlusion of the left subclavian artery and the brachiocephalic trunk, followed by a period of reperfusion. Following cerebral ischemia, a significant increase in blood flow (hyperemia) during the reperfusion phase has been observed using radioactive microspheres. Unilateral trigeminal nerve ganglionectomy diminishes this hyperemic response on the ipsilateral side. The purpose of this work is to evaluate dynamic susceptibility-contrast imaging at 4.7 T for detection and characterization of cerebral ischemia, hyperemic response during reperfusion, and the effect of trigeminalectomy. The preliminary data indicate that dynamic MRI measurement of hemodynamics during ischemia and reperfusion is in agreement with the temporal changes in blood flow measured by radioactive microspheres. This allows for the detection of ischemia based on changes in perfusion, as distinct from detection due to development of edema. Several factors must be considered in order to measure hemodynamic parameters by MRI. The most important is the ability to acquire images very rapidly. This requires the consideration of minimum echo time (TE) and pulse repetition intervals (TR). In addition, animal studies require strong gradients so that small fields of view may be used. These and other technical considerations required for successful MRI measurement of cerebral hemodynamics will be presented.
